1. Our Commitment to Your Privacy
Nexus Care is committed to protecting the privacy and confidentiality of your personal health information (PHI). We handle your information in accordance with Ontario's Personal Health Information Protection Act, 2004 (PHIPA) and Canada's Personal Information Protection and Electronic Documents Act (PIPEDA).
2. Information We Collect
We collect information you provide during account creation and health assessments, including your name, date of birth, contact details, province of residence, and responses to medical questionnaires. We also collect technical data such as IP address and access timestamps for security and audit purposes.
3. How We Use Your Information
Your personal health information is used solely to provide telehealth and pharmacy services: connecting you with licensed Canadian practitioners, processing prescriptions, fulfilling orders, and communicating with you about your care. We do not sell your information.
4. Consent
Before your information is shared with a practitioner, we obtain your express consent. You may withdraw consent at any time by contacting us, subject to legal and professional record-keeping obligations.
5. Disclosure to Practitioners
With your consent, your health information is disclosed to licensed Canadian practitioners and pharmacies within your circle of care for the purpose of treatment. Prescriptions are transmitted securely to dispensing pharmacies.
6. Safeguards
We protect your information with administrative, technical, and physical safeguards including encryption in transit and at rest, row-level access controls, session timeouts after inactivity, and detailed audit logging of access and authentication events.
7. Data Retention
We retain personal health information for the period required by applicable Canadian healthcare records legislation and professional regulatory requirements, after which it is securely destroyed.
8. Your Rights
Under PHIPA and PIPEDA you have the right to access your records, request corrections, and ask questions about how your information is handled. To exercise these rights, contact our Privacy Officer.
